What NAD+ is, and what it is not
NAD+ (nicotinamide adenine dinucleotide) is a coenzyme found in every living cell. It helps turn food into usable energy and supports DNA repair. It is not an approved prescription medicine. In the United States it reaches people three ways: as an oral dietary supplement, as a compounded injection, and as an intravenous drip sold by wellness clinics.
That status matters more than any marketing claim. Dietary supplements are not reviewed for safety or effectiveness before they go on sale, and the label claims are not evaluated by regulators, as the agency’s own consumer guidance on how dietary supplements are regulated explains. Injectable and intravenous versions are compounded, meaning a pharmacy mixes them to order. Compounded products are not FDA-approved and are not reviewed for safety, effectiveness or quality before sale, a point set out plainly in the agency’s questions and answers on drug compounding.
So when you read that a clinic offers NAD therapy, you are looking at a wellness service, not a treatment with an approved indication. That does not make it worthless. It does mean the burden of judgement sits with you and the clinician you choose. Read the rest of this page with that framing in mind, and treat anyone promising a cure as a warning sign.
Why a sunscreen label may appear on this page
Product databases match on brand names, and at least one cosmetic sunscreen uses the letters NAD in its product name. If you see a topical sunscreen label here, that is what it is: an over-the-counter sunscreen that helps prevent sunburn and, used with other sun protection, can reduce the risk of skin cancer and early skin aging. It is for external use only, should be kept out of the eyes, and should not go on damaged or broken skin. It has nothing to do with the supplements, shots or drips discussed below.
How the molecule works inside cells
Your cells run on chemistry that constantly moves electrons around. This coenzyme is the shuttle. It cycles between two states, an oxidised form and a reduced form, and that cycling lets cells break down food and produce ATP, the currency your muscles and brain spend all day.
It also acts as a raw material rather than just a helper. Three families of enzymes consume it: sirtuins, which influence how genes are switched on and off; PARPs, which help repair damaged DNA; and CD38, which sits in immune signalling. Every time those enzymes work, they use the molecule up. Cells then rebuild the supply through a recycling route called the salvage pathway, which reuses nicotinamide, one form of vitamin B3.
This is why the anti-aging pitch exists. Laboratory research suggests tissue levels shift with age, inflammation and metabolic stress, and animal studies of NAD-boosting compounds have shown effects on metabolism, muscle and heart tissue. Interesting biology is a reason to study something, not proof that a drip will change how you feel.
Is NAD just vitamin B3? The forms compared
Not quite, though the two are close relatives. Vitamin B3 covers several compounds your body converts into the coenzyme. Those compounds are called precursors. Most oral products sold for this purpose contain a precursor rather than the coenzyme itself, because the molecule is largely broken apart in the digestive tract before it can be absorbed intact.

Precursor labelling is worth checking closely. NMN in particular has had a bumpy regulatory ride: the agency has objected to its marketing as a dietary supplement, and the ingredient’s standing remains contested. What the research does and does not show
Here is the honest summary. Human trials of oral precursors show they can raise measurable NAD levels in blood. That part is fairly consistent. What those studies have not established is that raising a lab number reliably makes people stronger, sharper, thinner or longer-lived. A frequently cited review of NAD-boosting molecules and the human evidence describes exactly that mismatch between mechanism and outcomes.
Most published trials share the same limits. They are small, often a few dozen participants. They run for weeks or a few months, not years. They measure surrogate markers such as blood levels, blood pressure or insulin sensitivity, and results have been mixed. Fatigue, brain fog, athletic recovery, hangovers, long COVID and addiction recovery are the conditions clinics talk about most, and those are precisely the areas where controlled evidence is thinnest.
Intravenous use has even less behind it. Small studies and clinic case series exist, particularly around substance withdrawal, but there is no approved indication and no agreed protocol. Clinics that advertise otherwise are making claims that regulators expect to be substantiated; the standards for advertising health benefits are spelled out in the commission’s guidance on health product claims.
Does that mean nobody benefits? No. Some people report more energy and better sleep, and placebo effects with an hour-long infusion and attentive staff are strong. It does mean you should treat this as an experiment you are funding, set a time limit, and decide in advance what improvement would justify continuing.
Side effects reported with supplements, shots and drips
Effects differ by route, so it helps to separate them.
Oral supplements. Reported problems are usually mild and digestive: nausea, stomach discomfort, loose stools, bloating, headache, or trouble sleeping if taken late. Products containing nicotinic acid can cause a flushing reaction: hot, red, itchy skin, mostly on the face and chest, sometimes with a prickling feeling. Long-term safety beyond the study periods is simply not known.
Injections. Injection site reactions dominate: soreness, redness, a firm lump, bruising or itching. Some people feel briefly flushed, nauseated or lightheaded afterwards. Poor technique or a non-sterile product can cause infection, which is why the source of the vial matters as much as the ingredient.
Intravenous infusions. The pattern people describe is distinctive and closely tied to speed. Common reports include chest tightness or pressure, an urge to breathe deeply, stomach cramping, nausea, sweating, flushing, muscle twitching, a metallic taste and a jittery or anxious feeling. Slowing the drip usually eases it. Separately, any intravenous line brings its own risks: vein irritation, bruising, fluid leaking into surrounding tissue, and infection.
People often ask how long these effects last. Infusion sensations typically ease within minutes of the rate being reduced and fade after the session ends. Injection site reactions often settle over a day or two. Stomach effects from capsules usually improve after stopping. Symptoms that persist, worsen, or arrive with breathing trouble, fainting, a spreading rash or fever are not part of a normal reaction and need medical attention rather than a wait-and-see approach.
Warnings, interactions and who should be careful
Because NAD+ products sit outside the approved-drug system, they do not come with a standard interaction list. The clearer risks are attached to the B3 family, and those are well documented in the health professional summary on niacin intakes, safety and interactions.
- High-dose nicotinic acid can raise liver enzymes, raise blood sugar, raise uric acid and trigger gout flares, and drop blood pressure. Alcohol and hot drinks make flushing worse.
- Cholesterol medicines. Combining high-dose niacin with statins has been linked to a higher risk of muscle problems. This is a conversation for your prescriber, not a self-managed stack.
- Blood pressure and diabetes medicines. Added flushing, dizziness or shifting glucose readings can matter if you already take these.
- Liver or kidney disease. Get individual advice before starting any high-dose B3 or infusion product.
- Pregnancy and breastfeeding. Supplemental doses and infusions have not been studied for safety here. Prudence means avoiding them unless a clinician who knows your pregnancy advises otherwise.
- Cancer history or active treatment. NAD metabolism overlaps with how tumour cells fuel themselves, and researchers have not settled what boosting it means clinically. Raise it with your oncology team first.
- Compounded sterile products. Anything injected must be sterile and correctly concentrated. Ask which pharmacy prepared it and whether that pharmacy is a state-licensed 503A compounder or a federally registered 503B outsourcing facility.
One more practical warning: injectable vials sold online without any clinician contact, or shipped from overseas with no pharmacy named, are a poor bet on both safety and content. If a product arrives with no pharmacy label, you cannot know what is in it.
What NAD treatment costs when you pay yourself
This is where most searches land, and it is also where honest answers are hardest, because clinic drip pricing is set locally and changes often. We do not publish a figure we cannot stand behind, and infusion and injection pricing is not something the directory tracks the way it tracks monthly prescription programs. What we can do is explain exactly what moves the number.
- Route. Capsules are the cheapest entry. Self-injection kits sit in the middle. In-clinic infusions are the most expensive per session, because you are also paying for staff time and chair time.
- Session length and dose. Infusions are slow by necessity. Longer sessions and higher doses cost more, and clinics often price tiers by both.
- Who administers it. Nurse-led clinic visits, medical-spa sessions and mobile at-home drips carry different overheads. Travel fees are common for home visits.
- Add-ons. Vitamin blends, saline, glutathione, anti-nausea medicine and lab work are frequently billed separately.
- Packages and memberships. Single sessions usually cost the most. Bundles cut the per-session figure but lock in spending before you know whether you respond.
- Intro versus renewal. A first-visit rate is a marketing price. Ask what the standing rate becomes afterwards.
Before you buy a package, do the arithmetic on the whole year rather than the first visit, because a weekly or monthly schedule adds up faster than most people expect.
What a year actually costs
Programmes quote a monthly headline. Add the medication, the labs and the renewal price and the real number is usually different.
Advertised prices change often and intro pricing rarely lasts. Ask what the renewal rate is before you commit to a plan.
On coverage, be realistic. Insurers generally treat this as elective wellness care rather than medically necessary treatment, and services without an approved indication are the ones most often excluded. Never assume reimbursement, and never assume health savings account eligibility either. How clinic and telehealth programs differ
If you decide to try this, the program you choose changes your risk more than the ingredient does. These are the dimensions worth comparing side by side.
- Clinical review. Does a licensed clinician actually read your history, medication list and allergies, or is intake a checkbox form?
- Prescriber transparency. Can you find out who is responsible for the order, and can you reach that person with a question later?
- Labs. Some programs order baseline bloodwork, including liver function, and some order none. Neither is universal, so ask which applies and why.
- Pharmacy disclosure. For any injectable, the compounding pharmacy should be named, along with its licensing category and beyond-use dating.
- Training and support. Self-injection programs vary widely in the instruction, sharps disposal guidance and follow-up they include.
- State rules. Telemedicine prescribing is governed state by state, and licensure requirements shape who can treat you and what can be shipped where.
- Cancellation terms. Look for auto-renewal, minimum commitments and whether unused sessions expire.

When to stop and get medical help
Most reactions are mild, but a few need urgent care. Call emergency services or get to an emergency department for chest pain, trouble breathing, swelling of the lips, tongue or throat, widespread hives, fainting, confusion or a fast irregular heartbeat. Contact a clinician promptly for spreading redness, warmth, swelling or pus at an injection site, fever after a session, yellowing of the eyes or skin, dark urine, severe abdominal pain, or vomiting that will not settle. Stop the product and get advice rather than pushing through a symptom to finish a package you already paid for.
